Reminder checklist
Page 1- Please select a minimum of one property or program in which you wish to apply for. If you would like to apply for multiple programs or properties you may do so at this time.
Page 2- Work through the page of questions marking yes or no clearly. If prompted to complete a line for further information please do so.
Page 3- List all members that plan to be living in the household starting with the head of household. Filling out each line to the fullest.
Page 4- At the top of the page select yes or no clearly. The bottom half of the page is where you can list any employment, wages, or income (ex. Social Security, Child Support, Unemployment, etc.)
Page 5- At the top of the page note any Assets. The bottom half of the page is for Expenses this includes any handicap, medical, or childcare expenses your household currently has.
Page 6- In the Applicant Information area complete as much information as you can including name, address, phone number, etc. Don’t forget to check mark if you anticipate any changes in your household within the next year. If you have previous addresses please include them in the space provided
Page 7-List personal references and an emergency contact. List if you have any pets and how many. Select and complete the information for Veteran status. Sign the certificate of information.
Page 8- If you currently live or work in Clarion or Forest County complete the self certification of residency.
