Continuum of Care (COC)

The Continuum of Care (COC) Program is offered on a referral basis only. The COC Program has 21 beds split between transitional housing and permanent disabled housing. The goal is to help those consumers find helpful resources by providing stepping stones to evolve to a stable home life and stable financial state.
